At Diversified Dynamic Care, our Residential Placement Services provide compassionate, person-centered care for individuals with diverse and complex needs, including mental health disorders, acquired brain injuries, and intellectual or developmental disabilities.

Each home follows a structured staff-support model that offers 24-hour care to a small group of three to four residents, ensuring personalized attention and a nurturing environment.

Our goal is to help every individual live a meaningful, active, and fulfilling life. Residents are encouraged to participate in community outings, recreational activities, skill-building programs, and social events that promote inclusion, independence, and personal growth. We believe that every person, regardless of ability, deserves the opportunity to thrive and experience joy, connection, and purpose in their daily lives.

Each resident under our care receives an Individual Support Plan (ISP) developed collaboratively with them, their family, or an appointed advocate. These plans are reviewed regularly to reflect changing goals, abilities, and needs. The ISP outlines the supports and services we provide, as well as clear steps to promote physical, emotional, and social well-being. Focus areas include nutrition, physical fitness, hygiene, continuing education, emotional regulation, and personal safety.

As residents progress in building daily living and life-management skills, they may transition from a residential home to a mentor home setting, where they can experience greater independence while continuing to receive guidance and support. This transition reflects our commitment to empowering individuals to achieve their highest level of independence and quality of life.

Our professional team will hold your hands throughout your journey, offer support to you and your family, and provide you with a personalized care plan that reflects your wishes, the things, and the people that are most important to you. We will also help by connecting you with many other services that may be suitable for you and your family.
